New Patient Registration
We can only accept new patients who live in our practice area. You can register by completing the online registration form which can be found here. If you are unable to access and complete the form independently, our reception staff will be happy to help.
However, if you live out of the area but work in our catchment area you may be able to register for access to appointments and prescriptions. There are a few restrictions on follow up care after surgery or treatment and on the ability to make a home visit for example. Talk to us to find out more.
When you register, we will ask you to make an appointment to see either the practice nurse or healthcare assistant for a new patient health check.
This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
Once you have been accepted as a patient your medical records will be transferred to us.. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
If you have any medication on repeat prescription, we recommend ordering a months supply to ensure you are not left without during the transfer process.
