Unable to Access our Services Online?
We understand that not all of our patients are able to access our online triage form for various reasons—whether it's due to age, learning difficulties, or other circumstances.

Rest assured, you can still access our GP services. Simply call the practice as usual, and our reception team will be happy to complete the triage form for you.
For those who are able to complete the form online, we kindly ask that you do so. This will help save YOU time by avoiding long telephone queues and ensure that we can assist our most vulnerable patients more quickly.

Thank you for your understanding and cooperation

Total Triage – No NHS App? No Problem!
Need a GP appointment but don’t have the NHS App? You can still request one quickly and easily using the link below

Click the link above – you can also find it on our website:
www.lakesidepractice.co.uk . If you’ve consented to receive SMS messages, you may have also received this link via text.

Select 'I want help for a medical issue' – This option is available Monday-Friday from 8 AM. If it's not visible, we have likely reached GP capacity for the day.

Please do not submit an admin form for a medical issue requiring a GP appointment – it will be rejected, and you will be directed back to the correct form.

Confirm that your medical issue is not an emergency, then provide as much detail as possible when answering the questions.

Before submitting, you’ll be asked whether the request is for yourself or someone else – select the relevant option and enter the required details to ensure we process your request correctly.
Once submitted, a clinician will review your request, and we’ll contact you via your preferred method with an appointment or advice.

Appointments are often offered on the same day – If you can’t attend, please submit your request on a day when you’re available.

Need help? Let us know!

Total Triage Using the NHS App
If you need a GP appointment, you can complete our triage form using the NHS App by following these simple steps:

Log in to your NHS App
Click on the 'Services' tab (as shown below)

Select 'Contact your GP about a health problem'

DO NOT use the 'Check for available GP appointments' tab – all appointment requests are triaged by a clinician, so patients cannot book directly into an appointment slot.

Press 'Continue' on the next page to access the medical request form - This option is available Monday-Friday from 8 AM. If it's not visible, we have likely reached GP capacity for the day.

Confirm that your medical issue is not an emergency, then answer a few questions about your issue – please provide as much detail as possible

Before submitting your form, you’ll be asked if the request is for yourself or someone else – select the relevant option and enter the required details. This ensures we process the request correctly on our end.
Once submitted, your request will be reviewed, and we will contact you via your preferred method with either an appointment or advice from the triaging clinician.

Appointments are often offered on the same day. If you’re unable to attend that day, please submit your request on a day when you can come in if needed.

If you need help setting up the NHS App or completing the form, please speak to a member of our reception team.

IMPORTANT PATIENT INFORMATION – MOUNJARO

We have been receiving an increasing number of requests for the weight loss injection, Mounjaro.
Please be aware that weight loss injections are not currently available on the NHS. They are only offered by private providers, and you will need to contact them directly for access. Additionally, the private provider will determine whether the injection is safe for you to use.
If
you would like advice on diet and weight management, you are welcome to book a routine appointment with a nurse.
Thank you for your understanding.

RESPIRATORY SYNCYTIAL VIRUS (RSV)
From 1 September 2024, those who turn 75 and those age 75 to 79 will be eligible for a free vaccine to protect them from respiratory syncytial virus (RSV).
RSV is an infectious disease of the airways and lungs. RSV infection often causes symptoms similar to a cold. It can also make you become wheezy or short of breath and lead to pneumonia and other life-threatening conditions. Every year thousands of older adults need hospital care for
RSV, and some of them will die. RSV can be more severe in people with medical conditions such as heart or lung disease or a weakened immune system.
Pregnant Women are also eligible for this vaccine. RSV infection is common in young children but is most serious for small babies.
9 in 10 under 2s catch RSV. It can cause pneumonia and bronchiolitis. Every year thousands of babies need hospital care for RSV.
Vaccination from 28 weeks of pregnancy is the best way to protect your baby.
We will soon be inviting eligible patients to book their RSV vaccine appointment. Invitations will be sent by text message or through a letter in the post. Please keep an eye out for your invitation and book your appointment as soon as possible

Some of the comments made were;

Waiting times for telephone consultation call back
As an example, when booking a telephone consultation you may not be given an exact time but informed 'doctor will call this morning, after 10am'. We appreciate that for some patients such as those in work, this may be an issue if you are unable to use your phone during that time. If this is the case please make reception aware when booking so it can be noted it on the system. Our clinicians always aim to stick to given times where possible.
